If you are an existing patient and have had any changes to your contact or insurance information, please be sure to let us know at your next appointment so we can get a new copy of your driver’s license and/or insurance card(s).

Medical or Vision Insurance – Payment Options

We are pleased that you have chosen Atlanta Eye Care, Windward Eye Care and Alpharetta Eye Clinic where we do our best to accept all major insurance benefit plans. When booking your appointment for routine annual vision screenings we will ask for your vision insurance, also known as your vision vendor (EyeMed, VSP, Spectera). Be sure to bring your insurance card with you every time you visit Atlanta Eye Care. Please notify the front desk when your insurance coverage changes so that the medical billing staff can accurately maintain your important medical records. As a courtesy to our patients, we will file and track all claims and provide postage for special claims. Please be aware many insurance plans require patients to make a co-payment at the time of their visit. Atlanta Eye Care makes every effort to make each patient’s visit as easy and time efficient as possible.
